Qualifications

Responsibilities

• Conduct comprehensive health assessments and develop individualised care plans for participants in-home.
• Provide clinical nursing interventions, medication management, and wound care in home settings.
• Monitor client health outcomes and update care plans as needs evolve.
• Collaborate with support workers, allied health professionals, and family members to ensure safe care.
• Ensure clinical documentation is accurate and compliant with regulatory requirements.
