Job summary

EIH Care is seeking a compassionate, skilled Registered Nurse to support clients in their own homes across NDIS and Aged Care services. You will work autonomously, delivering person-centred care and contributing to clients’ independence and wellbeing.

Role emphasizes clinical duties, teamwork with families and health professionals, and adherence to care plans and regulatory requirements. Flexibility for casual shifts with a supportive team environment is offered.

Qualifications

• Current RN registration with AHPRA.
• Experience in community nursing, aged care or disability support.
• Strong clinical skills in wound care, continence management, catheter care, diabetes management and medication management.
• Excellent communication and interpersonal skills to build relationships with clients and families.

Responsibilities

• Conduct falls risk, continence and dementia assessments and implement care strategies.
• Provide clinical support with personal care while promoting dignity and independence.
• Support continence and urinary management including assessment and planning.
• Manage catheter care according to best practice guidelines and policies.
• Support diabetes management and implement individualized care plans.
• Assess, treat and monitor wounds with clear documentation.
