Job summary

Empower Healthcare is seeking a Physiotherapist to deliver high-quality care to participants in their homes across South-West Sydney. You will work within a supportive team, providing comprehensive assessments and tailored treatment plans under Support at Home and NDIS programs.

You will collaborate with other allied health professionals, educate participants and families, and manage records and patient reports, while maintaining professional standards and registration with AHPRA.

Qualifications

• Bachelor of Physiotherapy (or equivalent) required.
• Registration with AHPRA required.
• Police Check Clearance required.
• Excellent verbal and written communication skills required.

Responsibilities

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home through Support at home packages/NDIS.
• Work holistically with other allied health professionals to provide a multidisciplinary care plan.
• Educate participants and their families to optimise their ability to live healthy lives within the community.
• Liaise with service providers to ensure effective communication.
• Work autonomously to prioritise and complete tasks, maintain records and provide patient reports.

The Position

Our innovative service model focuses on delivering the best clinical outcomes for participants. As a Physiotherapist with Empower, you will have the opportunity to work with a variety of Musculoskeletal Neurological and Cardiorespiratory patients.

Responsibilities

Working in a supportive team environment, you will be responsible for the delivery of Empower Healthcare's comprehensive services across Support at Home and NDIS participants in South-West Sydney. You will:

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home through Support at home packages/NDIS.
• Work holistically with other allied health professionals to provide a multidisciplinary care plan
• Educate participants and their families to optimise their ability to live healthy lives within the community
• Liaise with service providers to ensure effective communication
• Work autonomously to prioritize and complete tasks, maintain records and provide patient reports

You will be provided with support from our home care team who will schedule these appointments and plan an efficient route.
