Job summary

At Home Together is seeking an experienced Occupational Therapist to deliver community-based OT services across Canberra and Melbourne. The role focuses on assessments, home modifications, and Assistive Technology for older adults and NDIS participants.

You will work independently from client homes, with some telehealth and interstate travel, supported by a multidisciplinary team, offering flexible part-time or full-time hours.

Qualifications

• Bachelor or Master’s degree in Occupational Therapy.
• Current AHPRA registration.
• NDIS Worker Screening Check, or willingness to obtain one.
• Working with Children Check where required.
• Strong written communication and report-writing skills.
• Excellent interpersonal skills.
• Ability to work independently in the community.

Responsibilities

• In-home Occupational Therapy assessments.
• Functional and environmental assessments.
• Assistive Technology assessments and recommendations.
• Coordinating and attending equipment trials with suppliers.
• Recommending home modifications (grab rails, ramps, etc.).
• Preparing clinical reports and documentation.
• Completing NDIS-related OT assessments and reports.
• Liaising with suppliers, builders, trades and other professionals.
• Telehealth assessments where appropriate.
• Travel between Melbourne, Canberra and Queanbeyan, including interstate travel.
