St Vincent’s Health Australia is seeking a Care Coordinator Occupational Therapist to manage a caseload and perform comprehensive assessments, care planning and support clients toward independence within their home and community. The role emphasizes evidence-based OT, motivational interviewing, and collaborative care planning.

The position is full-time and fixed-term until 8 October 2027, located in Fitzroy/CBD Melbourne area.

Qualificações

• Current registration with AHPRA as an Occupational Therapist.
• Minimum 2 years’ experience in relevant acute/community setting.
• Current Victorian Driver’s License.
• Experience with clients with multiple health issues, comorbidities and complex care needs that impact safety, function, independence and occupational performance.
• Experience delivering services in the community, addressing the needs of clients living with chronic diseases, aged related complex health needs, and psychosocial conditions.
• Current National criminal history check, or willing to obtain.

Responsabilidades

• Manage a caseload of clients and undertake comprehensive assessments, care planning and goal attainment.
• Provide high, quality, evidence-based occupational therapy assessment, intervention, education and management.
• Deliver services in clients’ homes and local community with goal of independence and safe functioning.
• Guide and support Allied Health Assistants in clinical delivery and program exit planning.
