Job summary
Northeast Health Wangaratta is seeking a part-time, ongoing DNS / HITH Registered Nurse to provide best practice nursing care in clients' homes or suitable environments. Duties include wound management, medication monitoring and education, diabetes care, catheter care, health assessments and palliative/home-based care within NHW’s integrated team.
Role involves shift work including weekends and public holidays; ongoing professional development and collaboration with the multidisciplinary team
Qualifications
- Registered Nurse with current registration and practising certificate.
- Experience in Community Nursing Programs or acute care is desirable.
Responsibilities
- Wound management and dressings as required.
- Medication monitoring, administration and patient education.
- Diabetes education and management for clients.
- Stomal and catheter care where needed.
- General health monitoring and client education.
- Health assessments, service advice and referrals.
- Hygiene assistance and assessment.
- Advanced care planning and palliative care in home-based care.
