Job summary

Northeast Health Wangaratta is seeking a Registered Nurse for the DNS/HITH program to deliver high-quality nursing care in clients' homes or suitable settings. The role covers wound management, medication monitoring and education, diabetes management, catheter care, and general health assessments as part of home-based care.

Work involves shift work, weekends and public holidays, with ongoing professional development and collaboration with the NHW multidisciplinary team to support community

Qualifications

Experience in Community Nursing Programs or clinical skills in assessment and care coordination in the acute sector.
Must have current registration as a Registered Nurse.
Ability to work in home-based care and with multidisciplinary teams.

Responsibilities

Provide wound management and medication monitoring, administration and education.
Support diabetes education and management.
Deliver stomal and catheter care and general health monitoring.
Perform health assessments, service advice and referrals as needed.
Assist with hygiene assessments and advanced care planning including palliative care and HITH.
