Job summary

My Community Health is seeking a talented Community Occupational Therapist to join our in-home nursing team servicing the Central Coast region.

You will assess clients, design customised treatment plans and deliver occupational therapy interventions to improve independence and quality of life. Join a supportive, growth‑oriented team and contribute to high‑quality, person-centred care for older Australians.

Qualifications

• Tertiary qualification in Occupational Therapy and current registration with AHPRA.
• Demonstrated ability to effectively communicate and build relationships with clients, families and teams.
• Excellent clinical reasoning, problem solving and commitment to evidence-based practice.
• Passion for promoting health, wellbeing and independence in the local community.
• Flexible and collaborative approach to working in a team.
• Australian working rights required.

Responsibilities

• Conduct thorough initial assessments and develop customised treatment plans for clients.
• Deliver a range of occupational therapy interventions and rehabilitation activities.
• Collaborate with clients, families and other health professionals for coordinated care.
• Maintain accurate client records and comply with policies and regulations.
• Participate in team meetings and ongoing professional development.
• Advocate for client-centered care and individual goals.
