Job summary

Uniting AgeWell is seeking a Community Enrolled Nurse to support our Home Care program in the West Melbourne Metro area. The role involves in-home assessments and collaboration with the Registered Nurse to ensure high-quality, client-centred care.

You will work three days per week during office hours, with a mix of in-office and home visits. Travel between clients is reimbursed; a Diploma of Nursing and current AHPRA registration are required.

Qualifications

• Diploma of Nursing with current AHPRA Registration (Div 2).
• 3+ years experience as Enrolled Nurse in Community/Home Care.
• Ability to visit clients in their homes and coordinate with RN/other professionals.

Responsibilities

• Conduct in-home clinical assessments and care reviews.
• Provide clinical advice and liaise with services for referrals as needed.
• Collaborate with local allied health professionals to facilitate responsive care.
• Operate in a permanent part-time capacity, 3 days a week during business hours.
• Balance in-office and home visit duties depending on client needs.
