Job summary

Uniting AgeWell Limited is seeking a Community Enrolled Nurse to support Home Care across South Eastern Melbourne and surrounds. You will conduct in-home assessments, work with the Community RN, and coordinate with allied health professionals to deliver person-centred care in clients' homes.

The role is permanent part-time (3 days/week) with a mix of in-office and home visits, in a values-based, supportive environment.

Qualifications

• Diploma of Nursing and AHPRA Div 2 Nursing registration are required.
• Minimum 3 years experience as an Enrolled Nurse in Community, CHSP or District Nursing.
• Team player with a flexible, positive approach.
• Experience visiting clients in their homes.
• Experience evaluating and implementing care support plans.
• Ability to exercise professional boundaries and sound decision making.
• Self-drive with own registered and insured vehicle (travel reimbursement provided).
• Commitment to the BEST values.
• Current Working With Children Check and flu vaccinations.
• Ability to pass a national police clearance and NDIS clearance.

Responsibilities

• In-home clinical assessments, reviews and care planning for clients.
• Provide clinical advice and liaise with services for referrals with RN support.
• Collaborate with local allied health professionals to enable responsive care.
• Permanent part-time, 3 days a week during business hours (may increase with demand).
• Balance office days with home visits as needed.
