Job summary

Uniting AgeWell Limited is seeking a Community Enrolled Nurse to support our Home Care program in the West Melbourne area. You will perform in-home assessments and care planning for clients, with supervision from a Community RN and liaison with relevant services.

The role is permanent part-time (3 days per week) during business hours, with travel to client homes and reimbursement provided for travel. A Diploma of Nursing and current AHPRA Div 2 registration are required.

Qualifications

• Diploma of Nursing with current AHPRA Div 2 nursing registration.
• Minimum 3 years experience as an Enrolled Nurse in Community/Home Care or District Nursing (desirable).
• Proven ability to work in a team and autonomously in client homes.

Responsibilities

• In-home clinical assessments and care needs reviews.
• Provide clinical advice and liaise with services for referrals.
• Collaborate with local allied health professionals to support in-home care.
• Permanent part-time 3 days a week during business hours; balance of in-office and home visits.
