Job summary

Lumia Care in Dulwich, SA is seeking a compassionate Enrolled Nurse (Care Partner) to deliver high-quality, person-centred in-home care, combining hands-on clinical tasks with ongoing wellbeing monitoring and documentation.

You will work within a collaborative clinical team, contribute to care planning, support independence for clients, and escalate concerns through established pathways, while maintaining accurate records and delivering safe, consistent care.

Qualifications

• Relevant qualification as Enrolled Nurse or equivalent in-home care setting.
• Current Police Clearance or willingness to obtain.
• Working With Children Check in Australia or willingness to obtain.
• NDIS Worker Screening Clearance or willingness to obtain.

Responsibilities

• Deliver personal care and in-home support in line with care plans.
• Monitor client wellbeing and escalate clinical concerns via escalation pathways.
• Contribute to care plan development and review with the clinical team.
• Maintain accurate, timely clinical documentation to support compliance and continuity of care.
• Identify and report incidents, hazards and safeguarding concerns.
