Job summary

Lumia Care invites applications for an Enrolled Nurse (Care Partner) to deliver high-quality, person-centred care in clients' homes across South Australia. This role blends hands-on clinical tasks with monitoring client wellbeing and accurate documentation to ensure safe, responsive support.

You will work within a supportive clinical team, contribute to care planning, and build trusted relationships with clients and families.

Qualifications

• Relevant qualification or experience in in‑home or personal care
• Current National Police Clearance (or willingness to obtain)
• Working With Children Check
• NDIS Worker Screening Clearance (or willingness to obtain)
• A valid driver's licence and reliable vehicle
• Evidence of current mandatory immunisations
• Full right to work in Australia

Responsibilities

• Deliver personal care and in‑home support in line with care plans, promoting independence and wellbeing
• Monitor client wellbeing and escalate clinical concerns, risks or deterioration via established escalation pathways
• Contribute clinical observations to support care plan development and review
• Support implementation of care plans, reinforcing safe, consistent and person‑centred practices
• Maintain accurate, timely clinical documentation to support compliance and continuity of care
• Identify and report incidents, hazards and safeguarding concerns
• Build trusted, professional relationships with clients, families and care teams
