Job summary

BaptistCare is seeking a Care Consultant for the Mornington region to support in-home care clients. You will assess needs, plan services, manage budgets and visit clients regularly to ensure care works in practice.

Experience in case management or allied health is valued; qualification in Case Management, Nursing, Allied Health or Social Work is required. You will work autonomously with a community-focused approach in a role that combines clinical foundations with compassionate, person-centred

Qualifications

• Qualification in Case Management, or an undergraduate qualification in Nursing, Allied Health, or Social Work (essential)
• Prior case management experience, particularly in home care or community services (highly regarded)
• Experience completing initial and annual customer assessments and care planning
• Knowledge of the Support at Home program and the broader home care sector
• Division 1 Nursing registration (desirable but not essential)
• Based in Mornington Suburbs and surroundings with a current driver's licence and willingness to travel across the region
• National Police Check (or willingness to obtain)
• Proof of working rights in Australia

Responsibilities

• Conduct intake, initial, and ongoing assessments of aged care-related needs
• Develop and maintain goal-directed care plans within each customer’s home care budget
• Coordinate and facilitate supports and services across the funded program
• Manage package spend and advocate for customers’ needs and preferences
• Conduct regular home visits and ongoing monitoring to support customers to age well in place
• Build strong relationships with customers, families, and community stakeholders
