BaptistCare is seeking a Care Consultant to support in-home care across Bendigo and surrounding areas. You’ll manage a caseload, assess needs, plan services, and ensure care is effective within the funded Home Care program.

The role offers full-time permanent employment, local community-based work, and autonomy in how you manage your caseload. Salary packaging is available to enhance take-home pay, with a focus on person-centred care and compassionate service.

Qualificações

• Qualification in Case Management, or an undergraduate qualification in Nursing, Allied Health, or Social Work (essential)
• Prior case management experience, particularly in home care or community services (highly regarded)
• Experience completing initial and annual customer assessments and care planning
• Knowledge of the Support at Home program and the broader home care sector
• Division 1 Nursing registration (desirable but not essential)
• Based in Bendigo area with a current driver’s licence and willingness to travel across the region
• National Police Check (or willingness to obtain)
• Proof of working rights in Australia

Responsabilidades

• Conduct intake, initial, and ongoing assessments of aged care-related needs
• Develop and maintain goal-directed care plans within each customer’s home care budget
• Coordinate and facilitate supports and services across the funded program
• Manage package spend and advocate for customers’ needs and preferences
• Conduct regular home visits and ongoing monitoring to support customers to age well in place
• Build strong relationships with customers, families, and community stakeholders
