Job summary

Silverchain is seeking a Care Partner to support older Australians in Kinglake West and surrounds. This full-time permanent role blends community visits with the flexibility to work autonomously from home, delivering personalised care in clients' homes.

You will assess needs, develop Care Plans, coordinate services and advocate for safe, independent living while building strong relationships with clients, families and multidisciplinary teams.

Qualifications

• Experience in community-based case management is highly regarded.
• Experience supporting clients in government-funded aged care services is advantageous.
• Strong communication and relationship-building skills.
• Demonstrated empathy and a client-centred approach.
• Effective problem-solving and organisational skills.
• Ability to work autonomously and as part of a multidisciplinary team.

Responsibilities

• Manage a caseload of older Australians receiving government-funded aged care services at clients' homes.
• Conduct client assessments and develop personalised Care Plans.
• Monitor client budgets in line with program requirements.
• Coordinate services to ensure timely and appropriate support.
• Support onboarding and ongoing service delivery for new clients.
• Build relationships with clients, families and key stakeholders.
• Collaborate with clinical, scheduling and service delivery teams to deliver coordinated care.
• Advocate for client needs to support positive health and wellbeing outcomes.
