Responsibilities

• Comprehensive Assessments of children, young people, and families to identify strengths, needs, and risks.
• Develop and implement tailored care plans in collaboration with clients and stakeholders.
• Provide crisis intervention to ensure safety and stability.
• Advocate for clients to access necessary resources and services.
• Facilitate evidence-based parenting programs and workshops.
• Collaborate with internal teams and external agencies to coordinate care.
