Job summary

South Eastern Sydney Local Health District (SESLHD) invites applications for a Registered Nurse in the Community Assessment Unit at Prince of Wales Hospital. You will deliver person-centred care, support comprehensive assessments, and coordinate nursing care within the MDT to achieve optimal patient outcomes.

The role requires current AHPRA RN registration, strong communication skills, and ability to work collaboratively in a busy hospital setting, with supervision of ENs and AINs where

Qualifications

• Registered Nurse with AHPRA registration.
• Experience in community assessment setting preferred.
• Good written and verbal communication skills.
• Ability to work in interdisciplinary teams.

Responsibilities

• Deliver high-quality, person-centred care in the Community Assessment Unit.
• Coordinate and implement nursing care with assessment, planning, intervention and evaluation.
• Maintain accurate clinical documentation and communicate with patients, carers and the multidisciplinary team.
• Supervise and delegate to Enrolled Nurses and Assistants in Nursing within scope of practice.
• Participate in quality improvement initiatives and ongoing professional development.
