Job summary

Clarity Health Care, in Queensland, is seeking social workers with mental health experience to provide counselling, assessments and tailored plans at the Medicare Mental Health Centre. You’ll work within a supportive, multidisciplinary team to improve participant well-being.

Responsibilities include intake assessments, collaboration with families, and advocacy to coordinate with clinicians and peers. A WWCC and police check are required; Medicare provider number is not necessary to start.

Qualifications

• Current AASW registration required.
• Experience in comprehensive mental health assessments and evidence-based interventions.
• Trauma-informed counselling and multiple clinical modalities.
• Ability to perform mental health triage and de-escalation.

Responsibilities

• Engage with participants offering personalised counselling, therapy and brief interventions.
• Conduct intake assessments and develop tailored counselling plans.
• Advocate for participants and coordinate with clinicians, peer workers and stakeholders.
• Monitor participants' mental health and wellbeing with health professionals as needed.
• Participate in clinical and case meetings, training sessions and professional development.
• Work in a multi-disciplinary team with peer workers and other clinicians.
