Job summary

Outcomes Connect Australia (OCA) is seeking a Behaviour Support Practitioner to join our collaborative team in Toowoomba, working with NDIS participants and their families. You will focus on enhancing quality of life, reducing behaviours of concern and promoting the rights of people living with disability.

You will work with clinical advisers, deliver evidence-based plans, and participate in ongoing PBS training with our partner institutions, while balancing flexible, remote/community-based work

Qualifications

• Current suitability (i.e. registration) with the NDIS Commission as a Behaviour Support Practitioner is preferred but not essential
• Considerable practical experience in supporting people with complex and challenging behaviours is essential
• Demonstrated ability to work positively in a diverse team
• Outstanding communication, influencing and interpersonal skills
• Current Driver’s Licence and own reliable vehicle

Responsibilities

• Identify needs through assessment and person-centred planning for NDIS participants and families
• Set meaningful goals and practical strategies using the Positive Behaviour Support Capability Framework
• Develop, deliver and evaluate evidence-based support plans within a complex space
• Ensure plans reflect clients' goals, passions and strengths
• Build knowledge, capacity and strengths of those who support clients (family, carers, friends)
