Job summary

HammondCare is seeking a permanent part-time Clinical Partner – Nursing to support clients living at home across the Central West, Nepean and Blue Mountains regions. This 0.6FTE role involves travel between Penrith, Hawkesbury, Upper Blue Mountains and Central West.

You will work with an experienced multidisciplinary team, conducting home visits and group activities, delivering person-centred care and setting realistic care goals to help clients stay active and independent.

Qualifications

• Registered Nurse with current AHPRA registration and practical experience.
• Experience in Aged Care, Inpatient, Community, Home-Based or Rehabilitation settings.
• Strong communication and time management skills.
• Able to work independently within a multidisciplinary team.

Responsibilities

• Assess functional care needs and set care goals.
• Provide treatment and intervention to support independence at home.
• Conduct home visits and run group activities for clients.
• Deliver person-centred care and contribute to continuous quality improvement.
