Job summary

HammondCare is seeking a permanent part-time Clinical Partner - Nursing to support clients living at home in the Central West/Nepean/Blue Mountains region. This role is three days per week, Monday to Friday, within the Home Care Clinical Care & Practice Team to assess needs, set goals and deliver tailored, person-centred care in clients’ homes.

You will work with an experienced multidisciplinary team to provide care, perform home visits or group activities, and contribute to continuous quality

Qualifications

• Registered Nurse with current AHPRA registration.
• Experience in aged care, inpatient, community or home-based settings is desirable.
• Strong communication and time-management skills.

Responsibilities

• Assess client care needs and set tailored care goals.
• Deliver person-centred nursing care in home settings.
• Coordinate with multidisciplinary team and document outcomes.
• Travel between client homes to provide home-based care.
