Job summary

HammondCare in Australia is seeking a part-time Clinical Partner - Nursing to support clients living at home in the Central West Region (Bathurst, Lithgow, Oberon, Cowra). This permanent role is 3 days per week, Monday to Friday, requiring you to drive between clients' homes.

As a Registered Nurse with current AHPRA registration, you will work with a multidisciplinary team to provide tailored and person-centred care, assess needs, set goals, and support independence for older adults living at

Qualifications

You're a Registered Nurse with current AHPRA registration or sound knowledge in aged care, inpatient, community or home-based settings.
Strong communication skills and ability to plan and manage time effectively.
Able to work independently as part of a multidisciplinary team.

Responsibilities

Provide home visits and tailored care to older adults in the home.
Assess functional needs, set goals and deliver treatment and interventions.
Travel between clients' homes in the Central West Region (Bathurst, Lithgow, Oberon, Cowra).
