Job summary

Disability Connect Victoria in Warragul is seeking an experienced Support Coordinator for a 15-month fixed-term maternity leave cover. The role is hybrid, combining office-based and remote work while supporting participants across the Gippsland region.

You will manage a caseload, help participants understand their plans, connect with providers, and document every step. The successful candidate will have strong communication, be well organised, and able to work independently within a supportive

• Experience in Support Coordination or a similar NDIS role.
• Good understanding of the NDIS and participant plans.
• Ability to manage own caseload and priorities.
• Strong communication and relationship-building skills.
• Organised and able to work independently.
• Good written documentation and computer skills.

Responsibilities

• Manage a caseload of NDIS participants.
• Support participants to understand and implement their NDIS plans.
• Connect participants with suitable providers and community supports.
• Build strong relationships with participants, families, providers and stakeholders.
• Monitor plan utilisation and participant outcomes.
• Assist with plan reviews and gathering supporting information.
• Maintain accurate case notes and documentation.
• Work collaboratively with internal and external stakeholders.
