Job summary

Centacare Brisbane seeks an experienced Clinical Assessor Delegate to deliver end-to-end aged care assessments within the My Aged Care framework. You will perform Comprehensive, Home Support, and SPRs, determine eligibility, and exercise delegated authority to finalise approvals and funding decisions.

This role is community-based with autonomy, prioritising workload, scheduling, and documentation, reporting to the Team Lead and collaborating with the broader team to meet service timeframes and

Qualifications

• Tertiary qualification in Nursing, OT, PT or SW with AHPRA registration.
• Minimum one year experience delivering aged care assessments.
• Completion of My Aged Care Workforce training and Assessor Delegate training requirements.
• Ability to independently manage a caselog and achieve KPI-driven outcomes.

Responsibilities

• Deliver end-to-end aged care assessments using the Integrated Assessment Tool (IAT).
• Triage and finalise service approvals and funding decisions in line with requirements.
• Maintain documentation accuracy and compliance with My Aged Care delegation.
• Contribute to audit, quality improvement and governance activities.
