Catholic Archdiocese of Brisbane – Darra QLD is seeking Clinical Assessor Delegates to deliver end-to-end aged care assessments within the My Aged Care framework, combining clinical expertise with strong operational performance.

Reporting to the Team Lead, you will undertake Comprehensive Assessments, Home Support Assessments and SPRs using IAT, determine eligibility, and finalise service approvals and funding decisions in line with legislative and contractual requirements.

Formación

• Tertiary qualification in Nursing, Occupational Therapy, Physiotherapy or Social Work with current AHPRA registration.
• Minimum one year experience delivering aged care assessments.
• Completion of, or capacity to complete, My Aged Care Workforce training and Assessor Delegate training.
• Demonstrated ability to independently manage a caseload and achieve quality and productivity outcomes in a KPI-driven environment.
• Strong clinical reasoning, documentation and decision-making capability.
• Confident communication and interpersonal skills with older people, families and multidisciplinary stakeholders.
• Strong digital capability and proficiency in Microsoft Office.

Responsabilidades

• Deliver high-quality, person-centred, culturally safe and trauma-informed aged care assessments across community and remote settings.
• Undertake Comprehensive Assessments, Home Support Assessments and SPRs using the Integrated Assessment Tool (IAT).
• Review, triage and progress referrals within KPI timeframes.
• Maintain an average productivity benchmark of two completed assessments per working day or other service benchmark as directed.
• Exercise approved Assessor Delegate authority to finalise service approvals and funding decisions.
• Maintain documentation accuracy and 100% compliance with My Aged Care delegation authority and program guidelines.
• Contribute to audit, quality improvement and governance activities.
