Job summary

Centacare Brisbane is seeking an experienced Clinical Assessor Delegate to deliver end-to-end aged care assessments within the My Aged Care framework. You will perform comprehensive assessments, triage referrals, and exercise delegated authority to finalise approvals while upholding regulatory and program requirements.

The role is community-based with a high degree of autonomy, requiring effective workload management, collaboration with the Team Lead, and adherence to KPI targets.

Qualifications

• Tertiary qualification in Nursing, Occupational Therapy, Physiotherapy or Social Work with current registration where applicable.
• Minimum one year experience delivering aged care assessments or working in geriatric services.
• Completion of or ability to complete My Aged Care Workforce training and Assessor Delegate training requirements.
• Demonstrated ability to independently manage a caselog and achieve quality and productivity outcomes in a KPI-driven environment.
• Strong clinical reasoning, documentation and decision-making capability.
• Confident communication and interpersonal skills with older people, families and multidisciplinary stakeholders.
• Strong digital capability and ability to use multiple systems and MS Office.
• Ability to obtain and maintain required licenses/certifications and work rights in Australia.

Responsibilities

• Deliver high-quality, person-centred aged care assessments across community and remote settings.
• Undertake Comprehensive Assessments, Home Support Assessments and SPRs using the IAT.
• Review and triage referrals within KPI timeframes.
• Maintain an average productivity benchmark (e.g., two completed assessments per day).
• Exercise Assessor Delegate authority to finalise service approvals and funding decisions.
• Maintain documentation accuracy and 100% compliance with delegation and program guidelines.
• Contribute to audit, quality improvement and governance activities.
