Job summary

Perth & Kinross Council is seeking a motivated social worker for the Hospital Discharge Team in Murray Royal Hospital, Perth. The role is full-time, with 36 hours per week, focusing on timely, safe discharges for adults and carers from age 16, in a supportive multi-disciplinary environment.

The successful candidate will carry out person-centred assessments, work within SDS, and uphold social work principles and values, with opportunities for training and development.

Qualifications

• Excellent interpersonal and communication skills.
• Effective time management and organisational skills for autonomous work.
• Ability to respond flexibly to crisis and competing demands.
• Solid knowledge of Self-Directed Support and its delivery.
• Strong links with team, partnerships and community.
• Knowledge of PKC services and demographics is advantageous.
• Commitment to outcomes with clients through person-centred assessments.

Responsibilities

• Complete Outcome Focused Assessments and Reviews.
• Negotiate flexible packages of care and monitor through SDS.
• Undertake statutory work under relevant acts.
• Demonstrate knowledge of social work principles and values.
• Engage in Adult Support and Protection Procedures and legislation.
