Icon Group's Slade Pharmacy Richmond at Epworth Hospital seeks a Grade 2 Clinical Pharmacist to provide comprehensive clinical pharmacy services, conduct reviews, and support therapeutic decisions across multiple specialties. You will work within a multidisciplinary team to deliver high‑quality, patient‑centred care in a dedicated hospital environment.

Qualifications

General registration as a Pharmacist with AHPRA
Bachelor of Pharmacy or equivalent
Experience dispensing in Australian or NZ community or hospital pharmacy
Strong communication and interpersonal skills
Ability to prioritise workload and work independently
Commitment to quality use of medicines and evidence‑based practice
Proactive learning and professional development

Responsibilities

Deliver clinical pharmacy services to allocated wards, including chart reviews and reconciliation
Provide evidence-based advice to medical, nursing and allied staff
Counsel patients and carers on prescribed therapies to promote adherence
Participate in clinical rounds and MDT meetings
Support safe and ethical use of medicines including investigational/compassionate supplies
Identify and resolve medication-related problems with prescribers
Contribute to medication safety initiatives and audits
Assist with training and supervision of interns, students and technicians
Maintain knowledge through ongoing education and development
