UPA Aged Care Solutions invites applications for a Home Care Worker in the Northern Rivers Region, based in Grafton. Deliver person-centred care per client plans, assist with personal care, medications, transport and respite services, and provide living support at home.

Ideal applicants hold a Certificate III in relevant fields, possess a current Australian driver’s licence, and have a reliable vehicle. Strong communication and teamwork skills are essential, with capacity to travel and use

We currently an opportunity for Home Care Worker to join our Northen Rivers Region team based in Grafton.

Your Responsibilities:

• Deliver individualised care using a person-centred approach as per client care plans
• Assist with personal care, medications, transport, and respite services
• Provide everyday living support such as domestic assistance, gardening, shopping, and meal preparation
• Complete accurate and timely reporting of client services
• Identify and report client risks via our incident management system

What You’ll Need:

• Certificate III in Individual Support, Aged Care, or Community Services (highly desirable)
• Current Australian Driver’s Licence and willingness to travel
• Registered and insured vehicle
• Compassion, empathy, and a passion for helping others
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Confidence in using technology
