Job summary

DPV Health in Melbourne’s northern region is seeking an experienced Support at Home Care Partner to develop and implement personalised care plans. You will review intake, budgets and connect participants with local services, ensuring person-centred care that aligns with preferences and independence at home.

The role collaborates with DPV Health teams, stakeholders, and referrers to uphold quality standards, budgets and statutory requirements.

Qualifications

• Diploma level or higher in community, aged care, health or related discipline.
• Minimum 1 year of extensive case management experience.
• Knowledge of Community Aged Care Services and Consumer Directed care.
• Experience with ACAT/RAS, Care Finders and other referrers.
• Strong budgeting, problem solving, and customer service skills.

Responsibilities

• Review intake information and establish care plans and budgets.
• Conduct participant visits with person-centred care planning and provide independent advice.
• Coordinate with DPV Health teams and partner service providers.
• Conduct annual reviews and adjust care plans within budget and quality standards.
• Ensure services are delivered ethically, legally and contractually.
• Work from multiple sites.
