Job summary

Immanuel Gardens Home Care in Buderim is seeking a compassionate Care Partner to support clients to live independently with dignity. You will build meaningful relationships, develop tailored care plans, and work with families to ensure services are person-centred and delivered within budget.

This full-time, permanent role offers a hands-on, structured environment with face-to-face visits when possible and video check-ins where needed.

Qualifications

• Experience in aged care, home care, or case management.
• Strong understanding of Home Care Packages, Support at Home and CHSP funding models.
• Demonstrated experience in care planning, assessments, and budget management.

Responsibilities

• Meet with clients to understand needs, goals and funding levels.
• Develop and manage personalised care plans aligned to preferences and budgets.
• Conduct regular client check-ins (minimum monthly).
• Review and adjust care plans to remain effective.
• Liaise with families and service providers to coordinate supports.
• Develop and monitor budgets and ensure services stay within funding.
