Job summary

Lutheran Services Australia is seeking a compassionate Care Partner for Immanuel Gardens Home Care in Buderim, QLD. You will help clients live independently with dignity, develop tailored care plans, and coordinate services with families and providers.

This hands-on, relationship‑focused role requires regular monthly check-ins, budget awareness, and accurate documentation to meet aged care standards while delivering person‑centred care at home.

Qualifications

• Experience in aged care, home care, or case management.
• Strong understanding of Home Care Packages, CHSP funding models.
• Experience in care planning, assessments and budget management.
• Excellent communication with clients, families and stakeholders.
• Person-centred, culturally safe and respectful approach.
• Strong organisational and documentation skills.

Responsibilities

• Meet with clients to understand needs, goals and funding levels.
• Develop and manage personalised care plans aligned to preferences and budgets.
• Conduct regular client check-ins (monthly) per government requirements.
• Review and adjust care plans to stay relevant and effective.
• Liaise with families, carers and service providers to coordinate supports.
• Develop and monitor budgets and ensure services stay within funding.
• Maintain accurate documentation and ensure compliance with aged care standards.
