Job summary

Mounties Care Home Care in Mount Pritchard is seeking a dedicated Care Partner to join our team on a full-time basis at our Mount Pritchard site. You will work with clients to help them stay as independent as possible at home and in the community, coordinating care and services and supervising our team of Carers.

Key duties include developing and coordinating client care plans, preparing budgets, assisting access to My Aged Care, visiting clients, meeting leads to sign them up, and participating

Qualifications

• Minimum 2–3 years of experience in a similar role.
• Current clear National Police Check.
• Current First Aid Certificate, including CPR.
• Demonstrated knowledge within the Aged Care sector.
• Demonstrated care coordination skills and ability to work effectively with clients, family members and services providers.
• NDIS knowledge desirable.

Responsibilities

• Develop and coordinate client care plans.
• Prepare individual client budgets.
• Assist clients to access My Aged Care.
• Visit clients in the community.
• Meetings with potential client leads and signing them up to our services.
• Participate in the after hours on-call roster arrangement.
• Maintain consistency and continuity in the clients care regime.
• Liaise with staff, clients, clients family and Healthcare providers where required.
• Maintenance of client confidentiality.
