Job summary

Australian Chinese Community Association of NSW – Chatswood NSW is seeking a Care Partner to join our Support at Home program. This full-time role in Chatswood focuses on helping clients live at home with choice, control and wellbeing.

You will plan care, collaborate with families and health professionals, and ensure culturally appropriate service delivery. Mandarin/Cantonese language skills and a driving licence are preferred.

Qualifications

• Qualifications in a related discipline (social sciences, health, psychology, social work, community services)
• Minimum 2–3 years' experience in Aged Care sector – especially Support at Home
• High level of computer literacy (Word, Excel, PowerPoint) and ability to learn new software
• Experience working with older people, including CALD backgrounds, with chronic/complex needs
• Possess insured motor vehicle with a current driver’s licence
• Able to speak Cantonese and/or Mandarin
• Australian citizen, permanent resident or visa with unrestricted working rights
• Willing to undertake a Nationally Coordinated Criminal History check

Responsibilities

• Undertake person-centred care planning using ACCA’s model, create care plans, budgets, schedules and set client service expectations
• Collaborate with clients, carers, families, advocates and other staff to maximise independence and community engagement
• Allocate tasks to care workers and ensure adherence to client care plans
• Ensure timely delivery of quality, culturally appropriate services
