Job summary

St Vincent’s Hospital Sydney, part of St Vincent’s Health Australia, is hiring a Registered Nurse for Hospital in the Home (HITH). This permanent full‑time role offers a 7‑day rotating roster with 0800–1630 or 1130–2000 shifts, delivering acute hospital‑level care to patients in their homes.

You must be registered with AHPRA, hold a NSW unrestricted driver’s licence, and have community healthcare experience.

Qualifications

• Currently registered as a Registered Nurse (division 1) with the Australian Health Professionals Regulation Agency (AHPRA).
• Current NSW unrestricted driver’s licence (class c or above).
• Experience working within community healthcare settings is highly desired.
• Demonstrated ability to work within an interdisciplinary team using effective communication and interpersonal skills.
• Demonstrated effective time‑management skills and ability to prioritise competing workloads within a complex environment.

Responsibilities

• Provide acute hospital‑level care to patients in their own homes as a safe alternative to inpatient admission.
• Assess, plan, implement, and evaluate care collaboratively to achieve health goals and outcomes.
• Lead and coordinate nursing and healthcare across settings, including referrals and collaboration with other professionals and services.
• Practice independently and collaboratively, maintaining accountability and appropriately delegating care based on staff education, training, and context.
