Job summary

NSW Health invites a Registered Nurse to join the Hospital in the Home (HITH) service at Macksville District Hospital. You will deliver safe, evidence-based, patient-centred care to people in their own homes and community, coordinating care with a multidisciplinary team to achieve positive outcomes.

You must be registered with AHPRA, bring strong clinical skills in HITH or community nursing, and excel in communication, assessment and problem-solving. A valid Australian driver licence is required.

Qualifications

• Current registration with AHPRA as a Registered Nurse.
• Experience in Hospital in the Home or community nursing.
• Sound clinical assessment and problem-solving skills.
• Excellent written, verbal and interpersonal communication skills.
• Ability to plan and coordinate patient care within a multidisciplinary team.
• Confidence managing patients with complex needs in the community.
• Commitment to patient-centred care and evidence-based practice.
• Current Australian driver licence.

Responsibilities

• Provide safe, evidence-based and patient-centred nursing in patients' homes and community settings.
