Job summary

NSW Health's St Vincent's Public Hospital in Sydney invites applications for a Registered Nurse (Year 2–8) to join the Hospital in the Home (HITH) team. This permanent full-time role provides acute hospital-level care in patients' homes, delivering evidence-based nursing across diverse communities.

You will work within an interdisciplinary team, manage care plans, and support referrals and collaborations with other professionals, while upholding NSW Health guidelines and local policies.

Qualifications

• Currently registered as a Registered Nurse (division 1) with AHPRA.
• Current NSW unrestricted driver's license (class C or above) to attend client's home.
• Experience working within community healthcare settings is highly desired.
• Demonstrated ability to work within an interdisciplinary team using effective communication and interpersonal skills.
• Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.

Responsibilities

• Deliver evidence-based nursing care to people of all ages and cultural backgrounds in patients' homes as part of Hospital in the Home (HITH).
• Assess, plan, implement, and evaluate care collaboratively to achieve health goals.
• Lead and coordinate nursing and healthcare across settings, including referrals and collaboration with other professionals.
• Practice independently and collaboratively, maintaining accountability and appropriately delegate care.
• Deliver care in line with NSW Health guidelines, policies and protocols.
