Job summary

Hospital in Your Home (HiYH) delivers hospital-level care in the place patients feel most comfortable: their own home. We partner with Queensland Hospital and Health Services to provide safe, high-quality care that reduces hospital stays and supports people to stay well and independent in their community.

As an Allied Health Assistant you will work under delegated supervision to deliver therapy programs and practical support in patients’ homes, enabling function, mobility and independence in the

Qualifications

• Certificate IV in Allied Health Assistance (or working towards it) or equivalent experience such as exercise physiology.
• Experience in a hospital, community, aged care or rehabilitation setting is desirable.
• Strong communication skills and a warm, patient-centered approach.
• Willingness to complete pre-employment checks, including a criminal history check and immunisation requirements.

Responsibilities

• Delivering exercise, mobility and functional programs prescribed by physiotherapists and occupational therapists.
• Supporting patients with falls prevention, transfers and daily living tasks.
• Assisting with equipment trials, set-up and patient and carer education.
• Monitoring patient progress and reporting changes to the treating clinician.
• Keeping accurate, timely clinical documentation.
• Working collaboratively with nursing, medical and allied health colleagues.
