Job summary

South Western Sydney Local Health District invites applications for an Allied Health Assistant to join the Transitional Aged Care Program (TACP). You will support therapy and care in clients’ homes, assist with home exercise programs and self-care retraining, and help with community access and transport to appointments.

The role works under Allied Health Professionals and Team Leaders within a multidisciplinary team, focused on reablement for older adults after hospital stays, across Fairfield

Qualifications

• Certificate IV in Allied Health Assistance or equivalent qualification.
• Minimum 12 months full-time equivalent relevant experience with commitment to undertake the Certificate IV in AHA within 12 months.

Responsibilities

• Deliver therapy and care in clients’ homes under supervision of Allied Health Professionals.
• Support interventions such as home exercise programs, self-care retraining, and community access.
• Provide transport support to appointments and assist with program administration.
