Job summary

South Western Sydney Local Health District invites applications for an Allied Health Assistant to join the Transitional Aged Care Program (TACP). You will support therapy and care in clients’ homes, assist with home exercise programs and self-care retraining, and help with community access and transport to appointments.

The role works under Allied Health Professionals and Team Leaders within a multidisciplinary team, focused on reablement for older adults after hospital stays, across Fairfield

Qualifications

• Certificate IV in Allied Health Assistance or equivalent qualification.
• Minimum 12 months full-time equivalent relevant experience with commitment to undertake the Certificate IV in AHA within 12 months.

Responsibilities

• Deliver therapy and care in clients’ homes under supervision of Allied Health Professionals.
• Support interventions such as home exercise programs, self-care retraining, and community access.
• Provide transport support to appointments and assist with program administration.

As an Allied Health Assistant, you will play an important role in delivering therapy and care in clients’ homes, supporting interventions such as home exercise programs, self‑care retraining, community access and transport to appointments. Working under the guidance of Allied Health Professionals and Team Leaders, you will be part of a multidisciplinary team committed to helping older adults achieve the best possible outcomes while maintaining their independence and connection to the community.

Selection Criteria

• Certificate IV in Allied Health Assistance or relevant qualification or Minimum 12 months full-time equivalent relevant experience with commitment to undertake the Certificate IV in AHA within 12 month period.
• Can you describe a time when you were delegated a task by a supervising Allied Health Professional that required you to manage competing priorities? How did you ensure you stayed within your scope of practice, met deadlines, and sought assistance when needed?
• In your previous roles, how have you adapted your communication style to effectively interact with both clinical staff and patients? Can you also share how you’ve applied Work Health and Safety principles in a healthcare setting?
• Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment.
