• Registered Nurse with AHPRA
• Experience in chronic disease management and complex care coordination
• Post graduate qualifications in relevant field
• Ongoing CPD in Chronic Disease Management

Responsabilidades

• Provide advanced nursing care to clients with chronic/complex health needs to maximise health outcomes
• Assist intake/triage, conduct assessments, coordinate goal-directed care in a multi-disciplinary setting
• Provide health literacy and self-help coaching in the community
• Support IPAC team in managing resources effectively
• Demonstrates use of Remote Patient Monitoring systems and devices
• Maintain KPI performance for the Complex Care program under HIP guidelines
