Western District Health Service in Hamilton, Victoria seeks a Credentialed ACOP on-site pharmacist to join the hospital pharmacy department. The role blends aged care and hospital practice across the full patient journey, offering exposure from inpatient care to residential facilities and opportunities for growth in medicines management.
Permanent full-time role (part-time considered) with relocation support and short-term accommodation.
Are you a Credentialed Aged Care On-site Pharmacist (ACOP) looking to broaden your clinical practice beyond residential aged care? Western District Health Service is offering a unique opportunity to combine aged care and hospital pharmacy practice within an innovative role that is helping redefine medicines management across rural healthcare. This position is ideal for pharmacists seeking variety, professional growth and the opportunity to work across the full continuum of care while remaining actively involved in residential aged care services. Unlike many ACOP positions embedded within community pharmacy, our model integrates pharmacists directly into the hospital pharmacy department, creating exposure to both hospital and aged care practice.
Our hospital-led ACOP model is already attracting national attention, with outcomes to be presented at Medicines Management 2026 through the abstract “Implementing a hospital-led aged care onsite pharmacist (ACOP) model in rural residential care.”
We understand that relocating for work is a significant decision, and we're committed to helping make the transition as smooth as possible.
Please note that accommodation support is offered as short-term shared accommodation only and is subject to availability.
Hamilton offers affordable living, minimal commuting, strong community connections and easy access to the Grampians National Park, Port Fairy, Warrnambool and Victoria's spectacular southwest coastline.
This unique approach enables pharmacists to develop expertise across the entire patient journey, from hospital admission through to residential aged care.
Deliver ACOP services across WDHS residential aged care facilities, supporting residents, families, nursing staff and medical practitioners.
Provide clinical pharmacy services to hospital inpatients and work collaboratively with prescribers, nursing staff and multidisciplinary teams to optimise patient outcomes.
Conduct medication reviews, identify medication-related risks and support evidence-based prescribing practices.
Contribute to medication safety programs, governance activities, audit processes and quality improvement initiatives across the organisation.
Support antimicrobial stewardship activities across hospital and aged care settings to promote safe and appropriate antimicrobial use.
Contribute to innovative service redesign projects and help shape the future direction of medicines management within WDHS.
Classification will be determined based on qualifications, experience and demonstrated capability.
This is an opportunity to join a department actively investing in innovation, service development and clinical excellence.
At Western District Health Service, we are committed to delivering high-quality, person-centred healthcare to communities across the Southern Grampians and Glenelg regions.
