Job summary

ForHealth Group is seeking a Registered or Enrolled Nurse to join the Dapto Medical Centre, focusing on chronic condition management and preventative care. You’ll work closely with GPs and a collaborative team to support patients and optimise care plans.

Qualifications

• AHPRA registration as EN or RN is mandatory.
• Experience in Chronic Condition Management preferred.
• Excellent communication and organisational skills.

Responsibilities

• Support and educate patients in managing chronic conditions and regular reviews.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams, including referral coordination.
• Manage CCM appointment administration.
• Coach patients on sustainable health behaviour changes.
• Promote preventive healthcare initiatives (heart checks, women's health, diabetes checks).
• Support CCM program growth with the practice team.
