ForHealth Group is seeking a Registered or Enrolled Nurse to join our Gladstone Medical Centre in Queensland. You will support GPs and patients through chronic condition management and preventative healthcare within a collaborative clinical team.

The role offers part-time or full-time flexibility, with opportunities to grow through training and progression. We value inclusive, patient-centered care and community wellbeing.

Qualificações

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
• Prior experience in Chronic Condition Management preferred.
• Strong communication and organisational skills are essential.

Responsabilidades

• Support and educate patients in managing chronic conditions and improving health behaviours.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams, including referral coordination.
• Manage administrative tasks related to CCM appointments.
• Coach patients on sustainable health behaviour changes.
