Job summary

Spearhead Supports Pty Ltd in Perth, Western Australia invites applications for a Disability Support Worker in a permanent full-time role. You will provide person-centred support to participants living with disability, helping them maintain independence and participate in home and community activities.

The position requires compassionate, reliable care, with a salary between $79,500 and $85,000 plus superannuation.

Qualifications

• Certificate III or IV in Individual Support or equivalent.
• Minimum 1 year of relevant experience in disability, aged care or community services.
• Current First Aid and CPR certification.
• Current National Police Clearance.
• Current NDIS Worker Screening Check, where required.
• Working with Children Check, where required for duties.
• Valid Australian driver's licence.
• Strong interpersonal and communication skills.
• Good documentation and record-keeping skills.
• Caring, respectful and person-centred approach.
• Ability to work independently and as part of a team.

Responsibilities

• Deliver person-centred disability support in line with individual plans.
• Assist with personal care and activities of daily living.
• Support mobility and safe transfers.
• Assist with meal prep, household tasks and routines.
• Provide medication assistance per procedures.
• Support community access, appointments and social participation.
• Assist with travel and transport.
• Promote independence, dignity and choice.
• Maintain progress notes and incident reports.
• Identify and report changes in participant needs or risks.
• Communicate with participants, families, colleagues and health professionals.
• Comply with WHS and safeguarding policies.
