Job summary

Connect2Care is actively seeking an Occupational Therapist for a community-based role in the South East of Perth. The position offers full-time or part-time hours with relocation support (interstate & international) and the option to work from home or hub locations across Australia.

You'll join a multidisciplinary team delivering evidence-based services via community, telehealth, and centre-based care, with a strong focus on NDIs goals and functional outcomes.

Qualifications

• Relevant tertiary qualification in Occupational Therapy.
• AHPRA registration.
• WWCC, NDIS Worker Screening and Police Check.
• Driver’s license and a reliable vehicle.
• Professional indemnity and public liability insurance.

Responsibilities

• Assess and treat clients with diverse needs (paediatrics + adults).
• Support NDIS goals and functional outcomes.
• Provide AT assessments and recommendations.
• Write clear, timely reports.
• Manage a varied caseload and meet clinical targets.
• Collaborate with families, referrers and other stakeholders.
• Engage in regular supervision and professional development.
