Job summary

SA Health’s Eyre and Far North Local Health Network seeks a Community Support Worker in Cowell. You will assist aged, frail or disabled individuals with personal care, meals, cleaning and transport, under Consumer Directed Care to support independence.

You’ll work with consumers, families and health professionals to create personalised care plans and provide emotional and social support to improve wellbeing and independence.

Qualifications

• Hold a valid driver’s license and have access to a reliable vehicle.
• Demonstrate dependable teamwork with clear communication skills.
• Show patience, resilience and a solution‑focused approach.
• Commit to consumer‑directed care and supporting independence.

Responsibilities

• Provide personal care, cleaning, meal preparation and transport.
• Collaborate with consumers, families and health professionals on care plans.
• Offer emotional and social support to improve wellbeing and independence.
• Help consumers achieve goals and preferences in a collaborative manner.
