Job summary

Uniting Victoria and Tasmania – Horsham is seeking an Early Help Practitioner to support children, youth and families with short-term, strengths-based interventions. The role covers Horsham and nearby communities, partnering with schools and community services to connect families with the right supports at the right time.

Qualifications

• Qualification in Social Work, Community Services, Psychology, Counselling or related discipline.
• Experience supporting children, young people and families through early intervention services.
• Knowledge of child development, family wellbeing and strengths-based practice.

Responsibilities

• Provide early intervention support to children, young people and families.
• Partner with schools, early years services and community organisations.
• Deliver parent coaching, family support and group-based programs.
• Coordinate referrals and connect families with local supports.
• Contribute to community activities that build inclusion and resilience.
• Support positive outcomes through collaborative, place-based practice in Horsham and surrounds.
