Job summary

CareChoice Group Ireland in Geelong, Australia, is seeking a dedicated Support Worker to provide person-centred support across SIL homes. You will assist with daily living activities, health supports, and community engagement, helping participants live as independently as possible.

The role requires Certificate III in Individual Support, 2 years with NDIS, and checks. You will work rosters, maintain records, and uphold dignity and choice while building trusting relationships.

Qualifications

• Minimum Certificate III in Individual Support (Ageing, Disability or related discipline).
• Minimum 2 years’ experience supporting NDIS participants with complex support needs.
• Experience supporting clients with Behaviours of Concern.
• Current NDIS Worker Screening Check and Working with Children Check.
• Current First Aid and CPR certification.
• Valid Victorian Driver’s Licence and access to a reliably insured vehicle.
• Availability to work the ongoing roster outlined above.

About The Role:

Join our Geelong Support Worker team and make a meaningful difference in the lives of people with disability. Based across our 24/7 Supported Independent Living (SIL) homes, you’ll provide person-centred support that empowers participants to live as independently as possible and achieve their individual goals.

Your Duties and Responsibilities:

• Provide tailored, person-centred support that promotes the participant’s independence, wellbeing, and quality of life.
• Assist with personal care, health supports, and day-to-day living activities in a respectful and professional manner.
• Perform manual handling and transfer duties safely, utilising prescribed equipment and following established support plans.
• Facilitate community engagement, appointments, and meaningful social and recreational activities.
• Build a positive and trusted relationship with the participant while upholding their choice, control, and dignity.
• Maintain accurate records and communicate effectively regarding any changes in the participant’s needs, health, or wellbeing.

About You:
